
С»ÆÑ¼ÊÓÆµ County Maternal Health Home Visiting Program provides nurse home visits to postpartum mothers and their newborns as well as to pregnant women with high-risk prenatal conditions. These services are provided with the goal of helping moms and babies have positive outcomes by providing personalized care in the privacy of the patients own home.
Postpartum / Newborn Home Visits
С»ÆÑ¼ÊÓÆµ County provides no cost postpartum and newborn home visits to С»ÆÑ¼ÊÓÆµ County residents who have Medicaid or no insurance. A onetime home visit is completed by a public health nurse between 1-2 weeks postpartum.
Services offered include:
- Health check for both mom and baby
- Blood pressure check for mom
- Weight check for baby
- Teaching and support
- Linking to community resources
- Assistance scheduling medical appointments
As a family in the postpartum period, education and support will include:
- Physical changes postpartum
- Health warning signs to be aware of postpartum
- Emotional changes/family support
- Baby care
- Breastfeeding assistance
- Family planning counseling
- Other medical follow-ups
- Help with any questions the family may have
Prenatal Home Visits for High-Risk Pregnancies
С»ÆÑ¼ÊÓÆµ County provides no cost skilled nurse home visits to С»ÆÑ¼ÊÓÆµ County residents who have Medicaid or no insurance and receive a referral order for home visits from one of our partnered providers.
Your personal public health nurse will provide a monthly health check during your pregnancy. This visit will include:
- Blood pressure check each visit
- Education and support tailored around you and your pregnancy needs
- Time to answer any questions you may have
- Collaborate with your provider
- Help connect you with any additional resources
Referrals and Contact Information
Our maternal nurse home visiting team welcomes your referrals for postpartum and newborn home visits. These referrals can be made by a provider, community member or self-referral.
