What is the Continuum of Care?
The С»ÆÑ¼ÊÓÆµ County Continuum of Care (CoC) is a community-wide effort to end homelessness in С»ÆÑ¼ÊÓÆµ County. We bring together local nonprofits, government agencies, housing developers, healthcare providers and people with lived experience to coordinate support, housing and services for those experiencing homelessness.
Goal: To ensure that homelessness is rare, brief and non-recurring in С»ÆÑ¼ÊÓÆµ County.
С»ÆÑ¼ÊÓÆµ County's Role as the Lead Agency
In 2024, С»ÆÑ¼ÊÓÆµ County Government became the official Lead Agency for the Continuum of Care. This means we are now responsible for leading, coordinating, and funding the local response to homelessness across three core areas:
1. Collaborative Applicant
We manage the annual application process for HUD funding and help ensure resources are used strategically across the community.
2. Homeless Management Information System (HMIS) Lead
We oversee the technology and data used by local agencies to track homelessness, support services, and ensure accountability.
3. Coordinated Entry Lead
We manage the process that helps people in crisis access housing and services through a fair and standardized system.
Our Progress So Far
Since stepping into the Lead Agency role, we've already made big strides:
- Signed a five-year agreement with the CoC to formalize responsibilities
- Hired a full-time director to lead the transition and build a strong team
- Secured key HUD grants and state funding to improve operations
- Gained support from partners like the City of Raleigh to support cold weather sheltering and long-term planning
What’s Ahead for 2026
We’re working on several priorities to strengthen our system:
- Hiring more staff to support Coordinated Entry and HMIS
- Launching a system-wide needs assessment to guide strategic planning
- Reviewing and upgrading our HMIS platform for better data and service coordination
- Reestablishing a call-in option for Coordinated Entry to improve access

The С»ÆÑ¼ÊÓÆµ CoC is made up of a broad and diverse network of partners:
- Nonprofit service providers
- Faith-based organizations
- Health and mental health agencies
- Public housing and education systems
- Law enforcement and local government
- People with lived experience of homelessness
Together, we plan, advocate and take action to end homelessness.
