Schedule a WIC appointment to find out if you are eligible for benefits. WIC services and appointments continue to be provided by phone or in person at this time Monday through Friday, 9 a.m.–5 p.m.; eWIC benefits will be issued remotely.

Before Your Certification Appointment, Have the Following Available:

Identification

Valid driver's license, Social Security card, current work/school ID, current Medicaid card, current military ID, birth certificate, or immunization record; and for infants: hospital crib card, ID bracelet or mother’s verification of facts form.

Residence

Current utility bill, valid driver’s license, current Medicaid card (or presumptive eligibility form), bank statement, current rental or mortgage receipts, or Division of Motor Vehicle ID card.

Household Income

Current paycheck stubs, if self-employed your recent tax return or history of earnings for the past twelve months, current Medicaid card, a letter of certification for the NC Food and Nutrition Services, a letter from your employer stating gross income and frequency of pay, or unemployment letter/notice.

Recent height/length, weight and hemoglobin

Measures from your provider or health record with the date, if available. If not available, WIC staff will let you know the next steps you need to take.
