What is Success Coach?

The Success Coach Program is a voluntary service to families that provides tailored, family-centered support to children and families who have achieved permanency through reunification, adoption, guardianship, or legal custody after foster care.  Success Coaches work alongside families to strengthen placements, reduce risks, and enhance child well-being that will result in long-term success for children and families.  Success Coach operates in 71 North Carolina counties, including С»ÆÑ¼ÊÓÆµ County, where it is administered by Children’s Home Society.

  • Success Coaches provide individualized, strengths-based services, including: Comprehensive assessments of family strengths and needs, personalized success plans, crisis support, and guidance on behavioral, mental health, educational, and financial challenges.

Eligibility

  • Must have at least 1 child in the home who is adopted through foster care system, private, or intercounty adoption OR reunified with birth/removal parent(s), achieved guardianship or custody from the foster care system, and currently lives in one of the counties we serve.
  • Child must be under 18 at time of service agreement.
  • Upon turning 18, adolescents who were previously enrolled in the Success Coach service can enroll in the program without parental involvement.
  • Though child has to be under 18 to initially enroll, once enrolled, the program can service them through age 21.  *Note: Until 21st birthday

How to Enroll

Why Partner with US?

Once a family voluntarily agrees to participate in the program, they are assigned a Success Coach who will assess their strengths and needs. Together, they will develop and implement a plan to support stability and safety in the home.

The Success Coach meets with families in their homes at least twice a month for a duration of up to two years. During these meetings, the Success Coach offers support, guidance, and resources to help the family navigate their new situation. Additionally, the program provides referrals to other agencies and practical assistance to further support the family's journey toward success.  A С»ÆÑ¼ÊÓÆµ County post-permanency social worker can partner with the Success Coach with a signed consent from the family.
